When you contact us, you will first speak to an Intake Specialist to pre-qualify the individual.
Required information we will need:
- Full name of patient
- Current behavior/reason for call
- Age and date of birth
- Address
- Patient history
- Legal status
- Financial information
If admission is appropriate, transport the individual to Park Royal Hospital’s Admissions Department with the below:
- Clothing for three days
- All medications or a written record of medications
- ***No valuables
